Precision medicine and molecular systems medicine (MSM) are highly utilized and successful approaches to improve understanding, diagnosis, and treatment of many diseases from bench-to-bedside. Especially in the COVID-19 pandemic, molecular techniques and biotechnological innovation have proven to be of utmost importance for rapid developments in disease diagnostics and treatment, including DNA and RNA sequencing technology, treatment with drugs and natural products and vaccine development. The COVID-19 crisis, however, has also demonstrated the need for systemic thinking and transdisciplinarity and the limits (...) of MSM: the neglect of the bio-psycho-social systemic nature of humans and their context as the object of individual therapeutic and population-oriented interventions. COVID-19 illustrates how a medical problem requires a transdisciplinary approach in epidemiology, pathology, internal medicine, public health, environmental medicine, and socio-economic modeling. Regarding the need for conceptual integration of these different kinds of knowledge we suggest the application of general system theory (GST). This approach endorses an organism-centered view on health and disease, which according to Ludwig von Bertalanffy who was the founder of GST, we call Organismal Systems Medicine (OSM). We argue that systems science offers wider applications in the field of pathology and can contribute to an integrative systems medicine by (i) integration of evidence across functional and structural differentially scaled subsystems, (ii) conceptualization of complex multilevel systems, and (iii) suggesting mechanisms and non-linear relationships underlying the observed phenomena. We underline these points with a proposal on multi-level systems pathology including neurophysiology, endocrinology, immune system, genetics, and general metabolism. An integration of these areas is necessary to understand excess mortality rates and polypharmacological treatments. In the pandemic era this multi-level systems pathology is most important to assess potential vaccines, their effectiveness, short-, and long-time adverse effects. We further argue that these conceptual frameworks are not only valid in the COVID-19 era but also important to be integrated in a medicinal curriculum. (shrink)

BackgroundThe SARS-CoV-2 pandemic has highlighted once more the great need for comprehensive access to, and uncomplicated use of, pre-existing patient data for medical research. Enabling secondary research-use of patient-data is a prerequisite for the efficient and sustainable promotion of translation and personalisation in medicine, and for the advancement of public-health. However, balancing the legitimate interests of scientists in broad and unrestricted data-access and the demand for individual autonomy, privacy and social justice is a great challenge for patient-based medical research.MethodsWe therefore (...) conducted two questionnaire-based surveys among North-German outpatients (n = 650) to determine their attitude towards data-donation for medical research, implemented as an opt-out-process.ResultsWe observed a high level of acceptance (75.0%), the most powerful predictor of a positive attitude towards data-donation was the conviction that every citizen has a duty to contribute to the improvement of medical research (> 80% of participants approving data-donation). Interestingly, patients distinguished sharply between research inside and outside the EU, despite a general awareness that universities and public research institutions cooperate with commercial companies, willingness to allow use of donated data by the latter was very low (7.1% to 29.1%, depending upon location of company). The most popular measures among interviewees to counteract reservations against commercial data-use were regulation by law (61.4%), stipulating in the process that data are not sold or resold (84.6%). A majority requested control of both the use (46.8%) and the protection (41.5%) of the data by independent bodies.ConclusionsIn conclusion, data-donation for medical research, implemented as a combination of legal entitlement and easy-to-exercise-right to opt-out, was found to be widely supported by German patients and therefore warrants further consideration for a transposition into national law. Before one can talk about global arrays and multimodal detection, one must be clear about the concept of information: How is it different from energy and how is it detected? And can it come to specify a needed movement? We consider these issues in our commentary.

The engineering-based approach of synthetic biology is characterized by an assumption that ‘engineering by design’ enables the construction of ‘living machines’. These ‘machines’, as biological machines, are expected to display certain properties of life, such as adapting to changing environments and acting in a situated way. This paper proposes that a tension exists between the expectations placed on biological artefacts and the notion of producing such systems by means of engineering; this tension makes it seem implausible that biological systems, especially (...) those with properties characteristic of living beings, can in fact be produced using the specific methods of engineering. We do not claim that engineering techniques have nothing to contribute to the biotechnological construction of biological artefacts. However, drawing on Descartes’s and Kant’s thinking on the relationship between the organism and the machine, we show that it is considerably more plausible to assume that distinctively biological artefacts emerge within a paradigm different from the paradigm of the Cartesian machine that underlies the engineering approach. We close by calling for increased attention to be paid to approaches within molecular biology and chemistry that rest on conceptions different from those of synthetic biology’s engineering paradigm. Multiple arousal theory suggests that there is more than one arousal system and that the activity of the multiple arousal systems can be observed using electrodermal measures at various body sites. The ideas expressed by multiple arousal theory are interesting but they do not constitute a theory. The absence of a specific definition of arousal and the lack of testable predictions prevent multiple arousal theory from constituting a useful theoretical framework for focussed empirical research.

This paper studies a decision maker who tackles a choice problem by selecting a subset of (at most) two alternatives which he will consider further in the second stage of his deliberation. We focus on the first stage where he chooses the delebration set. We axiomatize three types of procedures: (i) The top two: the decision maker has in mind an ordering and chooses the two maximal alternatives. (ii) The two extremes: the decision maker has in mind an ordering and (...) chooses the maximal and the minimal alternatives. In diesem Buch werden Aspekte der Aussagenlogik und der Prädikaten­ logik der ersten Stufe behandelt. Eine Mathematisierung und Kalkü­ lisierung der Logik kann natürlich ganz verschieden ausfallen, je nach dem, von welchen Motiven man sich primär leiten läßt. Wir stellen drei Gesichtspunkte, die uns auch als die wesentlichsten erscheinen, in den Vordergrund: Die Formalisierung des Wahrheits­ begriffes, die Formalisierung des Beweisbarkeitsbegriffes und das Problem des Suchens nach Beweisen. Diese drei Aspekte führen zu drei verschiedenen Arten von Kalkülen. Die Betonung des (...) Wahrheitsbegriffes führte auf die untersuchung der Hilberttypkalküle von einem Standpunkt, wie er etwa auch im Buch von Rasiowa-Sikorski [Ra-Si] eingenommen wird. Hierbei wurde besonderen Wert auf die algebraischen Techniken gelegt, denn die Natur der Vollständigkeitsbeweise in diesen Kalkülen läßt sich u. E. eigentlich nur algebraisch verstehen. Etwas überspitzt könnte man formulieren, daß die Vollständigkeitsbeweise in Hilberttypkal­ külen Korollare zu Betrachtungen über Kongruenzrelationen in ge­ wissen Boole'schen Algebren sind. Bei den modelltheoretischen Be­ trachtungen haben wir uns kurz ge faßt und nur einige grundlegende Begriffe vorgestellt. (shrink)
